In an article published from ZDNet (click here to read the complete article) an iPhone traveling to Sydney was seen to be "spewing black smoke" and was able to be extinguished by an extinguisher. After testing, it was reported that a screw loose managed to puncture the battery's casing. The screw is believed to have been left unattended inside the phone following an "botched screen-replacement job" by an unauthorized Apple repairer.
